Fall 2020 Fundraiser Donation Form

The purpose of the Johnston Band Parents Association is  to promote, encourage, and support the Johnston Community School District’s (JCSD) 5th-12th grade instrumental music students, directors, and their programs; to encourage parental involvement; to develop positive community awareness and appreciation of the Johnston Band programs; to offer financial support to the activities of the Johnston Bands that are beyond the resources of the JCSD, when such activities are within the JCSD policies and procedures.

Tax deductible credit card donations can be made to the Johnston Band Parents Association through this Johnston Booster Club donation form or by downloading the donation form and mailing it along with a check as instructed on the form.  Johnston Band Parents Association is registered as a 501(c)(3) organization.

Available donation amounts:

  • $25 - Provide incentives to Summit band students
  • $50 - Help send a student to band camp
  • $100 - Pay for a judge/clinician
  • $250 - Upgrade audio equipment
  • $500 - Buy a marching band uniform
  • $1000 - Buy a new instrument
